  2. Bring the paperwork

    Bring the blank form, your insurance card and your child’s shot record if you have it. If something is missing, come anyway.

  3. Leave with it signed

    The exam takes about twenty minutes. You walk out with the form completed, signed and ready to hand in.

Questions about physicals

How fast can my child get a sports physical?
Often the same day. During the August back-to-school rush it may take a day or two, so call (559) 424-0610 early in the day for the best availability.
Is a sports or school physical covered by Medi-Cal?
For children, school and sports physicals are generally covered as part of routine well-child care. We verify your coverage before the visit and tell you if anything would be owed.
Do you do DOT physicals for commercial drivers?
Yes. We perform Department of Transportation exams and provide the medical examiner’s certificate you need for your commercial license.
My employer gave me a form. Can you fill it out?
Yes. Bring the form to your appointment and we will complete and sign it during the visit.
What if my child is missing a required vaccine?
We can usually give the missing vaccine at the same visit and update the shot record, so one appointment covers both the physical and the immunization.
Do I need to be an existing patient?
No. New patients are welcome for physicals, and many families start with us this way.
