How to make us your Medi-Cal clinic

If your Medi-Cal is assigned to a different clinic, you can change it. You do not need a reason, and you do not need permission from the clinic you are leaving. It takes one phone call.

  1. Call us first

Tell us the plan name on your card and we will confirm we are in your network before you change anything. (559) 424-0610.

  2. Call your health plan

Use the number on the back of your Medi-Cal card — CalViva Health or Anthem Blue Cross — and ask to change your assigned primary care provider to Arya Medical Group in Mendota.

  3. Wait for it to take effect

Changes usually start on the first day of the following month. Your plan will confirm the date, and so will we.

  4. Come and see us

Once it is active, call and book. If you need to be seen before the change goes through, call anyway — we will work it out.

Help next door

AMOR Wellness Center, a few doors away

Their programs, not ours — but they are a short walk from our door at 115 Belmont Ave, open the same hours we are, and they help Mendota families with the things a doctor’s visit cannot fix. Call them directly at (559) 696-3408.

Common questions about coverage

Do you accept Medi-Cal at the Mendota clinic?
Yes. We accept Medi-Cal and we are accepting new Medi-Cal patients. Call (559) 424-0610 and we will verify your plan before your visit.
I have CalViva Health. Can I be seen here?
CalViva Health is one of the two Medi-Cal managed care plans in Fresno County. Call us with your card and we will confirm your coverage and get you scheduled.
How much will my visit cost with Medi-Cal?
For most Medi-Cal patients, covered primary care visits cost nothing out of pocket. We verify your benefits ahead of time and tell you if anything would be owed.
Do I need a referral to be seen?
For primary care with us as your assigned clinic, no referral is needed. If your plan has you assigned elsewhere, call us and we will walk you through changing it.
What if I do not have Medi-Cal or any insurance right now?
Call us anyway at (559) 424-0610 — and ask AMOR Wellness Center next door at 115 Belmont Ave. They help people apply for Medi-Cal and other public benefits every day. Eligibility rules change, so it is worth asking them rather than assuming you do not qualify.
Do you speak Spanish?
Yes. Our front desk, nursing staff and physicians speak Spanish, so you can handle scheduling, your visit and your follow-up entirely in Spanish.

Bring your card. We will handle the rest.

Call and we will check your coverage, answer your questions in Spanish or English, and find you an appointment — often the same day.
